The emergence of targeted radionuclide therapy has been one of the latest breakthroughs in cancer radiotherapy, with somatostatin analogs being the priorities. Some of the successfully used radiolabelled somatostatin analogs in cancer diagnosis and treatment include 68Da-DOTOTATOC and 177Lu-DOTATOC. On the other hand, the approval of everolimus by FDA for use in cancer treatment have been an important breakthrough as the medication has so far recorded positive reviews. This study sought to explore the combined use of everolimus and 177Lu-DOTATOC for the treatment of neuroendocrine tumours in a clinical study involving 9 patients. Peptide-based radiopharmaceuticals comprise of four parts that include a radionuclide, a targeting peptide, a bifunctional chelating agent and a linker moiety. the chelating agents used for labelling somatostatin analogs with radionuclides are based on DOTA. The most commonly used products of DOTA modifications are DOTATOC and DOTATATE, and they are produced through peptide conjugation of 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid. For DOTATOC synthesis, the process involves conjugating 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid with Tyr3-octreotide to form DOTA0-Tyr3-octreotide (DOTATOC) Both DOTATOC and DOTATATE have the ability to bind to somatostatin receptors, and thus they are important vehicles of targeted radionuclide therapy after their conjugation with suitable radioisotopes. Some of the examples of radiolabelled DOTATOC compounds that are of significance to this paper are 177Lu-DOTATOC and 68Ga-DOTATOC. However, unlike 177Lu-DOTATATE which has been studied significantly in relation to targeted radionuclide therapy for its importance in the treatment of neuroendocrine tumors, 177Lu-DOTATOC has not been as widely studied. In this study, the combination of everolimus with 177Lu-DOTATOC as a treatment regimen was investigated for the efficacy, immediate and long-term complications, and the pharmacological characteristics when used for treating metastatic and/or inoperable neuroendocrine tumours. The subjects received daily everolimus 10 mg and four cycles of (100mCi) 177Lu-DOTATOC in order to determine the antitumor effects of the regimen. This study sought to explore the combined use of everolimus and 177Lu-DOTATOC for the treatment of neuroendocrine tumours in a clinical study involving 9 patients. In this study, the combination of everolimus with 177Lu-DOTATOC as a treatment regimen was investigated for the efficacy, synthesis using a new fluidic module and the radicals might be induced by this combination using Electron Paramagnetic Resonance EPR. The regimen was found to be efficacious as indicated by improvement of patients Karnofsky scores from 66 percent to 88.5 percent, a long progression-free survival of 33.5 months, overall objective response rate of 59.3 percent, and 34.5 months’ duration of disease control. In addition the combined therapy produces minimal adverse events as well as safe biodistribution after administration. However, the synergistic effect of both drugs on Sulphur radical production should be further investigated by EPR using different radionuclides as well as external beam irradiation. Preparation of the radiopharmaceuticals and quality control were instrumental in the overall study outcomes.

Il crescente interesse per le terapie con radionuclidi è stata di fondamentale rilevanza nei recenti sviluppi per la personalizzazione dei trattamenti, con particolare interesse per gli analoghi della somatostatina. Tra gli analoghi radiomarcati della somatostatina usati con successo sia per finalità diagnostiche sia per la terapia ci sono 68Ga-DOTOTATOC e 177Lu-DOTATOC. D’altro canto l’approvazione da parte della FDA dell’everolimus nel trattamento delle neoplasie ha costituito un importante successo, viste le positive reazioni suscitate finora. Questo studio si propone di valutare l’uso combinato di everolimus e 177Lu-DOTATOC per il trattamento delle neoplasie neuroendocrine in un gruppo di 9 pazienti. I radiofarmaci peptidici si compongono di quattro parti fondamentali che includono il radionuclide, il peptide, un agente chelante bifunzionale, una porzione ligante. L’agente chelante utilizzato per gli analoghi della somatostatina è generalmente basato sul DOTA. I più comunemente utilizzati cono DOTATOC e DOTATATE ottenuti dalla coniugazione con 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid. Il processo di sintesi del DOTATOC richiede l’utilizzo del 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid con octreotide –Tyr3 a formare DOTA0-Tyr3-octreotide (DOTATOC). Sia DOTATOC che DOTATATE hanno la capacità di legare i recettori della somatostatina e, mediante opportune marcatura con radioniclidi per terapia, possono essere impiegati anche a scopo terapeutico. Esempi significativi sono 68Ga-DOTATOC usato a scopo diagnostico e 177Lu-DOTATOC usato a scopo terapeutico. Tuttavia mentre il 177Lu-DOTATATE è stato estensivamente studiato nelle sue applicazioni per la terapia delle neoplasie neuroendocrine, il 177Lu-DOTATOC non è stato ugualmente oggetto di tali valutazioni. In questo studio sono stati valutati l’efficacia, le complicazioni a breve e lungo termine del trattamento combinato di everolimus e 177Lu-DOTATOC nel trattamento di neoplasie neuroendocrine metastatiche od inoperabili. I pazienti arruolati hanno ricevuto 10mg/die di everolimus e sono stati trattati con 4 cicli di terapia radiorecettoriale con 3700 MBq (100mCi) di 177Lu-DOTATOC per ciascun ciclo. Per la sintesi del radiofarmaco è stato impegato un nuovo modulo fluido ed il monitoraggio dei radicali indotti dalla combinazione è stato eseguito mediante Electron Paramagnetic Resonance EPR. Il trattamento combinato ha mostrato una efficacia in termini di miglioramento del Karnofsky score da 66 percento a 88.5 percento, progression-free survival di 33.5 months, un overall objective response rate di 59.3 percento e 34.5 mesi di controllo della malattia. Inoltre tale trattamento combinato non sembra avere rilevanti effetti avversi e non determina alterazioni nella biodistribuzione del radio farmaco. Tuttavia l’effetto sinergico delle due terapie è meritevole di ulteriori valutazioni soprattutto mediante la EPR utilizzando anche differenti radiofarmaci e radioterapia a raggi esterni. La preparazione dei radiofarmaci ed i controlli di qualità sono stati monitorati per tutta la durata dello studio.
