Juvenile firoadenoma is a rare special form of fibroadenoma, larger than 5 cm, that arise in adolescence. The mass is generally smooth, rubbery, with pushing margins and caracterized by rapid enlargement. Histologically the stroma is more abundant and cellular than common fibroadenomas but the lobular component is less conspicuous. Clinical features and ultrasound aspect are the same of cystosarcoma phillodes which has both benign and malign form. In this article we report the case of a 13 years old patient who came to our Istitute with a late mass taking the most part of right breast. Clinical examination and breast ultrasound suggested the diagnosis of juvenile fibroadenoma but do not exclude a cystosarcoma pyllodes. She underwent local excision of the tumor with the widest margin in accord with the possibility of breast reconstruction without implant. Microscopic examination showed a juvenile fibroadenoma with desease free margins.
