Fifty two eyes affected by keratoconus and other corneal diseases underwent penetrating keratoplasty and were evaluated in order to observe the effect of some variables such as recipient-donor trephine disparity. A significant increase in corneal curvature was observed with 0.1 recipient-donor trephine disparity group if compared with the same size grafts group. Keratoconus patients showed a deeper anterior chamber and increased axial length and therefore higher myopic values after penetrating keratoplasty. According to our data we suggest the use of same size recipient and donor trephines in order to reduce the incidence of myopia after penetrating keratoplasty.
