Proton therapy enables highly localized dose delivery due to the finite range of protons and the presence of the Bragg peak. However, the conventional use of a fixed Relative Biological Effectiveness (RBE) of 1.1 does not fully account for the dependence of biological effectiveness on radiation quality, dose, and cell type. This thesis presents two cell-specific models for calculating photon isoeffective (IsoE) doses in proton therapy, developed within the framework of the Argentine Proton Therapy Center (CeArP) and international collaborations with Italy. The Photon Isoeffective Dose (PhID) model describes macroscopic radiation quality through LET and accounts for mixed particle fields, LET-dependent radiobiological parameters, damage repair, and synergistic effects. The Continuous Microdosimetric Photon Isoeffective Dose (COMPhID) model extends this approach by incorporating microdosimetric information through the first three moments of the lineal-energy distribution. Both models provide cell-specific IsoE doses and reproduce the dependence of biological effectiveness on radiation quality and dose. The results demonstrate the potential of IsoE-based approaches to provide a more biologically consistent description of proton therapy and support their future integration into advanced treatment planning.
La terapia protonica consente di ottenere una distribuzione della dose altamente localizzata grazie alla portata finita dei protoni nei tessuti e alla presenza del picco di Bragg. Tuttavia, l’uso convenzionale di un valore fisso di Efficacia Biologica Relativa (RBE) pari a 1.1 non tiene pienamente conto della dipendenza dell’efficacia biologica dalla qualità della radiazione, dalla dose e dal tipo cellulare. Questa tesi presenta due modelli cellulo-specifici per il calcolo delle dosi fotoniche iso-efficaci (IsoE) nella terapia protonica, sviluppati nell’ambito del Centro Argentino di Protonterapia (CeArP) e di collaborazioni internazionali con l’Italia. Il modello Photon Isoeffective Dose (PhID) descrive la qualità della radiazione a livello macroscopico attraverso il LET e tiene conto dei campi misti di particelle, dei parametri radiobiologici dipendenti dal LET, dei processi di riparazione del danno e degli effetti sinergici. Il modello Continuous Microdosimetric Photon Isoeffective Dose (COMPhID) estende questo approccio incorporando informazioni microdosimetriche attraverso i primi tre momenti della distribuzione dell’energia lineare. Entrambi i modelli forniscono dosi IsoE cellulo-specifiche e riproducono la dipendenza dell’efficacia biologica dalla qualità della radiazione e dalla dose. I risultati evidenziano il potenziale degli approcci basati sulla dose IsoE nel fornire una descrizione biologicamente più coerente della terapia protonica e nel supportarne la futura integrazione nei sistemi avanzati di pianificazione del trattamento.
