The development of progressive anemia and thrombocytopenia during the course of PV and ET, besides suggesting an evolution into SMF, might be also a surrogate of BMF3 grade, eventually resulting in a worse outcome. This suggests to monitor carefully cell blood count in PV and ET patients, with the aim to detect early a possible transformation into SMF.
