The paper examines two operations carried out almost simultaneously in the Kingdom of Sardinia and in the State of Milan starting from 1770, i.e. the restoration of the 15th-century hospital of San Matteo in Pavia and the construction of the new hospital of Sacramento in Vigevano. The two events are placed in chronological contiguity and share the broader scenario, Italian but above all European, of developing more rational and more efficient healthcare facilities, paying specific attention to the relationship with city planning. However, within parameters that touch similar historical contexts and theoretical debates, the two cases stand out for the different options adopted by government institutions.
