Paraneoplastic cerebellar degenerations (PCDs) associated to Hodgkin’s Lymphoma (HL) often show liquoral or serologic detection of antibodies against DNER antigen. Unlike what might be thought, ectopic expression of onco-neural DNER antigen by Reed-Sternberg cells has always played a marginal role in pathogenesis of these syndromes, according to previous literature. Here we report a case of HL with clinico-radiological evidence of PCD, serologic detection of anti–DNER antibodies, and ectopic cytoplasmatic expression of DNER antigen at level of Reed-Sternberg cells. This exceptionality could provide the basis for further investigation aimed at deepen the pathogenesis of these particular paraneoplastic neurologic syndromes. considering the complex HL-related microenvironment.
