This is the first description of transition from Hemicrania Continua (HC) to Short Lasting Unilateral Neuralgiform headache attacks with Conjunctival injection and Tearing (SUNCT) in a caucasic woman, 65 year old. This case, together with the other reports of transition from HC to chronic paroxysmal migraine, strengthes the suggestion of a continuity between HC and Trigeminal Autonomic Cephalalgia (TAC). HC is a continous headache with frequent exacerbations associated with cranial parasympathetic autonomic features. TACs share the clinical features of headache and prominent cranial parasympathetic autonomic features, with pain free in the intercritical period.
