Remarkable therapeutic progress has been accomplished in pediatric allergology in recent decades, driven by the advances in molecular biology and immunopharmacology and the understanding of the inflammatory pathogenic mechanisms in primary allergic conditions. Notably, a shared feature among diseases with different clinical manifestations, such as atopic dermatitis (AD) and allergic asthma, is a “Type 2 helper T cell” (Th2) response. Biologically active proteins targeting specific therapeutic pathways exemplify the paradigm of tailored and precision medicine, yielding successful outcomes, particularly in the context of severe allergic skin and respiratory diseases. Monoclonal antibodies are humanized or chimeric immunoglobulins generated in vitro from a single immortalized clone of B lymphocytes. Currently, only a select few of these biologics are approved for clinical use as anti-IgE (omalizumab), anti-IL5 (mepolizumab), and anti-IL4/13 (dupilumab) for pediatric and adolescent populations, while others are still in the clinical trial phase.The following sections outline the main and updated applications of biological drugs in various allergic diseases.
