The results of this study suggest that TPOAb in the first trimester of pregnancy have deleterious effect on surrogate measures of placentation such as first and second trimester Doppler pulsatility index of uterine arteries, increasing the risk of restricted fetal growth and preeclampsia. Both stratified analysis and logistic regression support an independent effect of TSH concentration and TPOAb positivity on uterine Doppler pulsatility and fetal growth without any significant interaction. Testing for thyroid autoimmunity in the first trimester of pregnancy could be useful in selected population, as women with history of obstetric complications, recurrent miscarriages and other autoimmune diseases
